December is National Drunk and Drugged Driving (3D) Prevention Month.
The Kentucky Transportation Cabinet, Kentucky Office of Highway Safety, would like to recognize
those law enforcement officers and agencies from across the state who have saved lives
through their extraordinary efforts to reduce impaired driving.
Call for Nominations
Nomination are being accepted through November 3. The awards recognize law enforcement officers and agencies from across the state who have saved lives through their extraordinary efforts to reduce impaired driving. Presentations will be made at a luncheon on Tuesday, December 16, 2014, at the Embassy Suites, 1801 Newtown Pike, Lexington, Kentucky.
REGISTRATION: 10:15 a.m. (EST) CEREMONY: 11:00 a.m. (EST)
NOMINATION DEADLINE: Monday, November 3

How to submit your nomination
All nominations must be received no later than Monday, November 3.
Notification of award recipients
Individuals and agencies selected for the awards will be contacted by email or phone using the information submitted on the nomination form. If you do not receive a confirmation by November 28, contact Lauren Germond, email@example.com or 859-257-7412, as we may not have received your nomination.
Registration will begin at 10:15 a.m. and the awards ceremony will follow promptly at 11:00 a.m. EST.
NOTE: Due to reserved seating, there are no seats or meals for additional guests. The cost of the luncheon is covered by the Kentucky Transportation Cabinet, Kentucky Office of Highway Safety and it is intended for law enforcement personnel only. If you are unable to attend, please notify us a minimum of three (3) business days in advance.
It is requested that uniforms be worn.
Overnight accommodations can be made by calling the Embassy Suites at 859-455-5000 or 1-800-EMBASSY. A block of rooms identified as the "Governor's Impaired Driving Enforcement Awards" is available for a rate of $119.00 single/double plus tax until November 16. Lodging is the responsibility of the individual.
